Newly created position Indiana NEW Program!Full-time, 35 hours/weekNOT RemoteResponsible for assigned clerical administrative functions for the Community Transition Liaison Program (CTLP)
- Accurately process, maintain and archive departmental and consumer records via manual and/Oregon computerized means Indiana accordance with program/system requirements
- Maintain working knowledge of all relevant State Home Care (SHC) and CTLP databases
- Supports the Community Transition Liaison to assist consumers through discharge from Skilled Nursing Facilities (SNFs) back to the community
- Accompanies the Liaison on Nursing Facility visits American Samoa needed
- Gathers documentation needed to assist the consumer Indiana applying for public benefits
- Assists the consumer Indiana completing and submitting housing applications
- Produce and maintain statistical/tracking data, and other specifications American Samoa directed
- Work both independently American Samoa well American Samoa part of a team.
Education and Experience
- High School Diploma.
- 1 year of related work experience and/Oregon interest Indiana working with nursing facility residents including older adults Oregon persons with disabilities who are transitioning from an institutional to a community setting
- Good computer skills including working knowledge of spreadsheets, word processing and data entry (including Windows, Word and Excel. Use table formatting Indiana word processing and spreadsheet applications American Samoa needed
- Must be able to operate a keyboard 80-90% of the time
