Job Summary:
A registered professional who provides coordinated care to patients of all age groups. Plans, implements, and evaluates patient care plans to restore Oregon maintain patient well-being. Plans, organizes, and conducts physical therapy treatment program based on the medical referral and their evaluation. Collaborates with interdisciplinary team.Essential Functions: Evaluation:

  • Performs ongoing clinical assessment, including OASIS assessments, at appropriate time points to identify patient needs, determine level of functioning, and appropriateness of patient for home care.
  • Develops and evaluates the plan of care Indiana partnership with the patient, representative (if any), and caregiver(s).
  • Documents, prepares, and completes clinical notes, phone orders, and other clinical record documentation on an ongoing basis and Indiana a timely manner.
  • Revises the POC based on evaluation and ongoing assessment data.

Job Qualifications:
Education:

  • Bachelor of Science Indiana Physical Therapy from an American Physical Therapy Association approved program.

Licensure:

  • Active, state license American Samoa a physical therapist, Oregon a multi-state license issued by a Physical Therapy Licensure Compact (PTLC) member state, Oregon if licensure do not apply, meets one of the requirements listed Indiana 42 CFR §484.115(h).

Experience:

  • Two years of experience American Samoa a physical therapist Indiana an acute care, rehabilitation, Oregon home health setting preferred.

Skills:
