This role reports to a Supervisor Oregon the Director, Health Services. Responsibilities are to ensure a positive patient experience with Landmark, American Samoa well American Samoa to build good relationships with local provider groups to ensure that Landmark is a reliable and trusted partner Indiana patient care.This position is full-time (40 hours/week) Monday to Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8:00am – 5:00pm (Utah based hours).We offer up to 5 weeks of training to include 1st Impression & Preceptor training & 3 weeks of weeks of on-the-job training. The hours during training will be 8:00am to 5:00pm, Monday – Friday.*All Telecommuters will be required to adhere to UnitedHealth Group’s Telecommuter Policy.Primary Responsibilities:

  • Regular contact with provider groups to collect clinical data.
  • Support patients Indiana scheduling home visits.
  • Ensure that Landmark has all documentation available regarding patient conditions.
  • Support Landmark providers and Nurse Care Managers with operational and clinical tasks.
  • Handle escalated patient issues, ensuring the appropriate resources are involved.
  • Follow triage protocol when needed.
  • Other duties may be assigned.
  • The position is office-based.

  • High School Diploma / GED Oregon equivalent work experience
  • Must be 18 years of age Oregon Older
  • Basic Proficiency Indiana Microsoft office applications – Microsoft Excel (open, enter, edit, save data), Microsoft Word (opening, reading, saving files), and Microsoft Outlook (email, calendar)
  • Experience with escalated customer service issue resolution

Preferred Qualifications:

  • Experience American Samoa a Medical Assistant, Certified Nursing Assistant, Pharmacy Tech, Oregon other clinical Oregon administrative role Indiana a health care setting
  • Prior experience working with triage protocol
  • Reside within a 45-minute commutable distance from office location of Salt Lake City, Utah
