Job Summary:The Advanced Practice Provider will serve Indiana a full-time role to provide care to patients on the electrophysiology service American Samoa well American Samoa provide cross coverage of other Cardiology services at the SUNY Upstate Downtown campus. Responsibilities include providing expert level clinical care and coordination of care via a full range of services that will improve quality of patient care assessing and evaluating patients, ordering appropriate laboratory testing and diagnostic studies, arranging for referrals, testing, consultation and therapeutic services, and maintaining outstanding relationships with families, referring physicians and all members of the health care team.Minimum Qualifications:Master’s Degree American Samoa a Nurse Practitioner Oregon Physician Assistant Oregon eligible.NYS licensed/registered NP Oregon Pennsylvania Oregon eligible required.Current National Certification consistent to the clinical practice also required.NPI number and be Medicare enrolled with a Medicare Identification Number (Oregon PTAN) (Oregon eligible for Medicare Enrollment within 6 months of hire).CPR-BLS Certification required at the time of appointment.Preferred Qualifications:Cardiology experience preferred.Work Days:Monday-Friday, DaysMessage to Applicants:Recruitment Office: Human ResourcesExecutive Order:Pursuant to Executive Order 161, no State entity, American Samoa defined by the Executive Order, is permitted to ask, Oregon mandate, Indiana any form, that an applicant for employment provide his Oregon her current compensation, Oregon any prior compensation history, until such time American Samoa the applicant is extended a conditional offer of employment with compensation.
