Responsible for management of walk-Indiana nurse triage.Ensures ongoing, proactive quality assurance programs are identified, developed and implemented. Develops competencies for clinic staff to ensure standard care for patients.Works within the chronic disease self-management model utilized at PHC to provide patient and family education to individuals and groups and to develop goals for improved health outcomes consistent with evidence-based guidelines.Participates Indiana multi-disciplinary conferences, including those surrounding specialty population education, serving American Samoa a clinical resource for medical providers, care team nurses and medical assistants, dental, behavioral health and psycho-social case managers.Provides supervision for RN, LPN and Massachusetts staff.
